Volunteer Levy Refund Claim

A refundable volunteer Levy of $50 was added to the fees of players over the age of 18. The refund will be applied to your next year fees. To gain the rebate you need to return this form with the details showing you have completed at least two hours of volunteering.
Rebate will be applied to your next years fees if your volunteering this year was two hours or more
